Your application should include

• Name, surname and signature (for written applications)
• National ID number (foreign nationals: nationality and passport / ID number)
• Address for notifications
• E-mail and phone, if any
• Subject of the request

Evaluation

Your application will be concluded free of charge as soon as possible and within 30 days at the latest. We may ask for additional information to verify your identity.
